How Home Care Works and What to Expect
Once Mobility Plus Home Health Care receives the physicianís order for home care, we call the patient with the names of the clinicians that will contact the patient to set-up a comprehensive home visit, making our services immediately available to the patient. Our goal is to initiate the skilled care necessary for the improvement of the patientís specific health conditions within 24 to 48 hours of receiving the referral.
The initial home care visit is 1-2 hours in length. During this time, patients should expect the nurse or physical therapist to perform a complete head-to-toe comprehensive physical assessment. The patient should be prepared to share their complete medical, physical, social and mental health history, past and present. The patient should have all prescribed and over-the-counter medications, including supplements, available for review by the clinician on the day of the initial visit.
Clinicians will perform a home safety evaluation. Functional ability within the home will be assessed, including physical hazards such as stairs, rugs, placement of cords and potential trip hazards. Clinicians will evaluate the patientís cognitive ability to live safely within the home.
The goals for subsequent visits are determined by the findings of the initial assessment, including duration of care, treatment plans and patient education. Plan of care and treatment plans are adjusted during the course of treatment depending on the patientís progress or any other contributing health factors.

What Distinguishes Them?
Different home care agencies provide varying types of services. Mobility Plus Home Health Care is a Medicare certified home health agency that provides skilled medical care within the home. Services provided include: skilled nursing, physical therapy, speech therapy, occupational therapy, medical social work and certified home health aides.
How Much Do Home Care Services Cost?
Every person is different, especially when it comes to health and what type of home care is needed. The good news is that most insurances cover home health care services at 100%. Once Mobility Plus Home Health Care receives your physicianís order for home care, our insurance specialists will verify patient coverage and determine the amount of the deductible of the services or the co-payment that will be requested before treatment begins.
Who Should the Patient Call and Can He or She Access Mobility Plus Home Care at Any Time?
The patient can access Mobility Plus Home Health Care 24 hours a day 7 days a week by calling 1866-978-9850. The patientís needs and concerns will be addressed in a timely manner.
What Are Your Hours and Are Services Available Weekends and Nights?
Our office hours are Monday through Friday from 8:30-5:00 p.m. There is an on- call service after hours. Our clinicians see patients 7 days a week. If a patient contacts us in the middle of the night, we will assess the patient issue over the phone. If the patient circumstances cannot be handled over the phone and the patient is in distress we will advise the patient to seek emergency medical treatment.
Do Mobility Plus Staff Have Training in What They Do?
What Kind of Training?
All clinicians are professionally certified and licensed through the state of Michigan. We do provide continuing training throughout the year for all clinicians and there is a training process that we provide our clinicians before they go out and see patients.
Do Mobility Plus Staff Have a Supervisor?
Yes, Mobility Plus has a Clinical Supervisor and Quality Assurance Supervisors that oversee every patient case. Supervisors are in continual collaboration with the patientís case managers. All supervisors are accessible to the patient should there be a need.
How Does Mobility Plus Choose Who Will Come to a Home?
The physicianís order determines what disciplines go into the home, unless the opening clinician goes into the home and notes that an additional discipline should be added to help the patientís overall treatment success. Mobility Plus will contact the patientís physician before adding any discipline that is not on the initial order from the patient physician. Our clinical supervisor and intake coordinator will decide which clinician will be assigned to the individual patient case based on the patientís particular needs.
